My Life has multiple claim forms depending on the type of claim.
Attached are the ones that we use most often, should there be a claim that does not fall within these forms, please contact us for the correct one.
All of these must be submitted with the below mentioned documents, in the case of a disability or critical illness I will need the medical reports/ scan/ blood tests as well as the history of the insured prior to inception.

- Certified copy – Death certificate
- Certified copy – Deceased`s ID
- Certified copy – Claimant`s ID
- Certified copy – Marriage certificate- claimant
- Certified copy – Full birth certificate
- Certified copy – Claimant`s bank confirmation letter
- Certified copy – Medical Certificate / or post-mortem report
